A death extract is a certified true copy of the Death Certificate (DC).

ICA's Registry of Births & Deaths is the custodian of all death records which occurred in Singapore since 1872. All applications for a certified extract of these records have to be made on the requisite application forms. Applications can be made in person or via the Internet.
