Death Extract Application
A death extract is a certified true copy of the Death Certificate (DC).
ICA's Registry of Births & Deaths is the custodian of all death
records which occurred in Singapore since 1872. All applications
for a certified extract of these records have to be made on the
requisite application forms. Applications can be made in person
or via the Internet.
As part of our efforts to promote eGovernment to better serve the
public, application for death searches and/ extracts records from
law firms, can only be submitted through eXtracts
Online with effect from 18 September 2004.
Eligibility
Under the Registration of Births and Deaths (RBD) Act, any person
may apply for a certified death extract, as long as he is able to
furnish the necessary information and upon payment of the prescribed
fees.

Procedure
Application in person
- Submit application at:
Citizen Services Centre

- Proceed to obtain a Q-number and extract application form.
- Complete extract application form and wait to be called.
- Applicant will be issued with receipt for collection.
Application via Internet
- Applicant must complete the application form at eXtracts
Online.
- Submit the form and payment electronically to the Registry of
Births & Deaths.

Peak Period to
Avoid
The Registry of Births and Deaths is generally at its busiest on
Mondays, Fridays and Saturdays and on days before and after public
holidays. The peak hours are after 11 am on weekdays and Saturdays.
While every effort will be made to attend to each application as
quickly as possible, waiting times may be longer during these periods.
The public is advised to avoid coming to the Registry during these
peak periods, if possible.

Documents
Required (if application is made in person)
- Applicant's identity card;
- If the applicant is not holding a Singapore identity card, a
certified copy of his/her official identification document (e.g.
identity card, passport, driver's licence, etc) must also be submitted.

Fees
S$30 (incl GST) per death extract.
Search Fees
If the Death Registration Number is not furnished, applicants are
required to pay a search fee:
- S$35 (incl GST) for a 1-month search period
- S$35 (incl GST) for every additional one month search period
or part thereof
This is in addition to the extract fee that needs to be paid
after the record is found.
For Internet applications, please note that you are required to
have an e-mail account and a VISA or Mastercard Credit or Mastercard
Debit Card, for submitting your application via eXtracts Online.

Processing Time
The processing time for the Death Extract is approximately 4 working
days upon receipt of your application. Some cases may take a longer
time. For such cases, we will inform you when the extract is ready
for collection.
For a search of death record, we will inform you of the search
result once it is known.

Collection
Local applicants
- You may choose to receive by post or by collection at our service counters.
- For postal requests, ICA will not be liable for any replacement should it arrive crumpled.
Overseas applicants
- You will receive the certified death extract by post
Authorising someone to collect the certified death extract
(proxy)
- Bring letter of authorisation stating the collector's name and
identification number.
- The collector's identification document (IC/passport) and the
official payment receipt. Without official payment receipt, the
collector must furnish the application reference number.
